Fill the Annexure-I declaration and the specimen publication matter for publishing a change of name in the Telangana Gazette.
Give Annexure-I to the Mandal Revenue Officer, who issues Annexure-II. Then send the publication matter (two copies) with the enclosures listed on the last page to the Commissioner of Printing, Hyderabad.
